Details of cancer diagnosis and treatment

Please prepare documents that describe how you discovered the cancer and the details of the treatment you received.

• Examination: time of receiving various examinations; type of examination; examination results; name of cancer after diagnosis.

• Surgery: time of surgery; type and method of surgery. The name of the medical institution where the surgery was performed, and the pathological results after the surgery.

• Chemotherapy: duration of anticancer agent treatment; frequency of administration; type of anticancer agent; effectiveness of treatment.

• Radiotherapy: time, intensity, radiation dose, and exposure range of radiotherapy.

 CT, MRI, PET-CT, endoscopy, ultrasound, etc. (preferably DICOM standard). If you have information that can help you understand your current general condition, please bring it.

Please provide color photos of endoscopic images.
